Abstract:
Objective
To investigate the clinical efficacy and traumatic reaction of single-port video-assisted thoracoscopic surgery (VATS) in the treatment of non-small cell lung cancer (NSCLC) .
Methods
Seventy patients with NSCLC admitted to Department of Thoracic Surgery , Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine from January 2015 to October 2018 underwent radical resection of lung cancer . They were divided into single-port VATS group and multihole VATS group according to different surgical methods ,with 35 cases in each group . The following clinical data including blood volume , operation time , extubation time , satisfaction score , incidence of complications ,WBC count ,C-reactive protein (CRP) level and cellular immune function (CD 4+/CD8+T lymphocyte subsets) were compared between the two groups .
Results
The patients with single-port VATS had less bleeding ,shorter operation time and extubation time ,and higher satisfaction score than those with multi-hole VATS ( P<0 .05) .On the 1st and 7th day after operation , the WBC count and CRP value of the patients in the single-port VATS group were much lower than those in the multi-hole VATS group( P<0 .05) ;and the ratio of CD4+/CD8+T lymphocyte subsets of the patients in the singleport VATS group were much higher than those in the multi-hole VATS group( P<0 .05) .The VAS score of pain on the first day and the seventh day after operation was significantly lower in the single-port VATS group than in the porous VATS group ( P<0 .05) .The total incidence of complications in singleport VATS group was 8 .57% , and that in porous VATS group was 31 .43% . The difference was statistically significant (χ2 =14 .9331 , P<0 .01) .
Conclusions
Single-port VATS surgery for NSCLC is a treatment with clinical significance and high safety ,as the body would has less traumatic stress reaction ,less complications ,less pain ,and faster recovery ,which is worthy of clinical application .
