Abstract:
Objective
To investigate the clinical effects of tubeless and double-lumen tube intubation anesthesia in uniport video-assisted thoracoscopic surgery of sympathectomy for palmar hyperhidrosis .
Methods
Sixty-two patients with palmar hyperhidrosis undergoing sympathectomy were divided into two groups randomly . Twenty-nine patients were enrolled as an observation group and received operation under tubeless intravenous anesthesia with spontaneous breathing .Carbon dioxide was injected into the pleural cavity to form artificial pneumothorax during the operation . The other 33 patients were enrolled as a control group under double-lumen tube intubation anesthesia . Contralateral ventilation was performed during the operation .The two groups received T 3 and T4 sympathectomy by uniport video-assisted thoracoscopic surgery .The perioperative indicators differences of the two groups were analyzed and compared by statistical method .
Results
There was no significant difference in operation time , blood loss , the lowest oxygen saturation , the degree of palmar temperature rise ,the incidence of postoperative pneumohemothorax and compensatory hyperhidrosis between the two groups( P>0 .05) . Compared with the control group , preoperative anesthesia duration and postoperative recovery time were shortened in the observation group ( P<0 .05) .And the incidence of nausea and vomiting ,pharynx and larynx discomfort ,stimulating dry cough were less in the observation group ( P<0 .05) .The hospital stay and hospitalization cost in observation group were less than those of the control group ( P<0 .05) .
Conclusions
The tubeless VATS anesthesia for sympathectomy in the treatment of palmar hyperhidrosis is safe ,feasible and reliable and could be used as a priority method .
