目的

回顾性分析接受机器人辅助支气管袖式肺叶切除术患者的生存资料,研究该术式对于治疗中央型非小细胞肺癌(NSCLC)的肿瘤学效果。

方法

纳入2014年10月到2019年6月于青岛大学附属医院胸外科接受机器人辅助支气管袖式肺叶切除术的中央型NSCLC患者85例,患者一般信息和相关临床资料的收集来源于医院电子病例,通过患者门诊就诊记录和电话随访收集患者的生存信息。

结果

在85例患者的研究队列中,患者年龄(61.8±9.7)岁,其中,男性患者较多(84.7%),且大部分患者有吸烟史(76.5%)。术后30天和90天死亡率均为0%,所有病例经术中冰冻病理证实为R0切除,淋巴结清扫数目为(23.7±9.1)枚,淋巴结清扫站数为(6.7±1.1)站。对于病理分期,35例(41.2%)患者为病理Ⅰ期,病理Ⅱ期患者25例(29.4%),病理Ⅲ期患者25例(29.4%)。中位随访时间为61.0个月,5年总生存率(OS)为77.0%,5年无病生存率(DFS)为70.0%。肿瘤的左右侧在OS(P=0.822)和DFS(P=0.433)中差异均无统计学意义,手术切除上肺叶和下肺叶在OS(P=0.874)和DFS(P=0.597)中差异均无统计学意义。

结论

在R0切除的中央型NSCLC患者中,机器人辅助支气管袖式肺叶切除术的长期肿瘤学效果是满意的。

Objective

To investigate the oncological effects of robotic-assisted bronchial sleeve lobectomy in the treatment of centrally located non-small cell lung cancer (NSCLC) by retrospectively analyzing the survival data of patients undergoing robotic-assisted bronchial sleeve lobectomy.

Methods

A total of 85 patients with centrally located NSCLC who underwent robotic-assisted bronchial sleeve lobectomy in the Department of Thoracic Surgery of The Affiliated Hospital of Qingdao University from October 2014 to June 2019 were reviewed. The general information and relevant clinical data of the patients were collected from the hospital electronic records. Survival information of patients was collected through outpatient clinic visit records and telephone follow-up.

Results

In the study cohort of 85 patients, the mean age of the patients was (61.8±9.7) years. Among them, the majority of patients were male (84.7%) , and most patients had a history of smoking (76.5%) . The mortality rate at 30 days and 90 days after operation was 0%. All cases were confirmed by intraoperative frozen sections as R0 resection. The number of lymph nodes removed was 23.7±9.1, and the number of dissected lymph node stations was 6.7±1.1. For pathological stage, 35 (41.2%) patients were pathological stage Ⅰ, 25 (29.4%) patients were pathological stage Ⅱ, 25 (29.4%) patients were pathological stageⅢ. The median follow-up time was 61.0 months, the 5-year OS rate was 77.0%, and the 5-year DFS rate was 70.0%. There was no significant difference in OS (P=0.822) and DFS (P=0.433) between the left and right sides of the tumor, and the surgical resection of the upper and lower lobes also showed no significant difference in OS (P=0.874) and DFS (P=0.597) .

Conclusion

The long-term oncological outcomes of robotic-assisted bronchial sleeve lobectomy were satisfactory in centrally located lung cancer with R0 resection.
